Notice of Privacy Practices (NPP)

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
1. Our Legal Duty
Creating Consciousness Therapy PLLC is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with notice of our legal duties and privacy practices. We are required to abide by the terms of this notice currently in effect.
2. Specialized Protections (Addiction & Recovery)
In compliance with the February 2026 Part 2 Final Rule, records related to Substance Use Disorder (SUD) treatment receive heightened protection. We will not disclose SUD records to law enforcement or in legal proceedings without your express written consent or a specific court order that meets federal requirements.
3. How We Use and Disclose Your Info
Treatment: To provide and coordinate your healthcare.
Payment: To bill you or your insurance (e.g., Aetna, Highmark).
Healthcare Operations: For internal quality reviews or business management.
Mandated Reporting: We are required by Pennsylvania law to report suspected child abuse, elder abuse, or if you are a danger to yourself or others.
4. Your Rights
Access: You can request to see or get an electronic copy of your medical record.
Amendments: You can ask us to correct your record if you feel it is incorrect.
Right to a Breach Notification: You will be notified in the event of any breach of your unsecured PHI.
5. Good Faith Estimate (No Surprises Act)
Under the law, healthcare providers must give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
You can ask for this estimate at least 1 business day before your service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
6. Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at admin@creating-consciousness.com or with the Secretary of the U.S. Department of Health and Human Services.
